They were kinda old so I revised them and decided to put them here as a download released under the GNU Free Document License.So there it is below, you can download it as an .ods file which can be opened with Open Office (I hate M$ Office and don’t use it), a pdf file, and I also exported it as a .xls file but I don’t know how good it will look in M$ Excel since it wasn’t created with it. Make sure the layout is set as landscape, and if you use the .ods file make sure the margins are set for 0.13″ for the top and bottom and 0.5″ for the left and right, before printing it out.
